To refer an individual to the Tallapoosa Mental Health Court program, open and print a copy of the Tallapoosa Mental Health Court Referral form. Upon completion, scan/upload and submit the form via email to Ms. Glanton at bglanton@polkga.org. Please note, the referral must be completed in full; any missing information will result in delays in the processing of the referral.

Those referred must have a mental health diagnosis, along with a history or pattern of substance use or abuse, or a co-occurring disorder diagnosis. 

Applicants are screened for legal eligibility requirements, and will be evaluated for clinical appropriateness for the program.
